What are the best GUARANTEED COVERAGE Health Insurance companies?

What are the best Health insurence companies that provide plans for seriouse pre-existing conditions(for my brother).  I want to move to any of the following states for coverege:
Florida, Califonia, Nevada,  Oregon, Washington.

Secondly, I know it is expensive, but how much can I expect to pay for full coverege.  I would like a plan which I can select my own doctor without having to be refered to one.

Depending upon the type and severity of the condition chances are your brother will be declined with any individual plan. You'll need to contact an independent agent that works with all the major companies licensed in the states you are interested in. The agent can contact the insurance company underwriters for a pre-screen, which can give you information on which companies may accept him and on what terms. There is no extra charge using an agent.

Companies can accept you but place a rider on the condition (which means they won't cover that condition), cover the condition with an increase in premium, cover the condition after a certain time period (which varies by state) without an increase of premium, or outright decline to cover at all. For this reason it's impossible to give you even a ballpark idea of premium. His age will also be a factor. In my state a healthy 20 year old can get a policy ranging from $45 to $500 per month while a healthy 50 year old will pay from $200 to $900 per month.
